Duties
Help
  • Coordinate healthcare services available at Military Treatment Facility (MTF).
  • Conduct a comprehensive assessment of beneficiary's health needs to develop a plan of care.
  • Serve as liaison between the patient and the health care professionals.
  • Coordinate referrals for each patient under case management services.
  • Assess patients with various complex medical conditions daily.
  • Complete follow up assessments in the electronic record with updated information of the patient's current physical and emotional condition.
  • Responsible for the integration of Utilization Management, Case Management, and Disease Management based on the Military Health Systems (MHS) Medical Management Model.
  • Serve as the first level supervisor performing supervisory responsibilities.

  • This position is designated Tier 3, Non-Critical Sensitive, Moderate Risk and requires the incumbent to obtain and maintain a Secret Security Clearance Level as a condition of employment.
  • One year probationary period may be required.
  • Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position.
  • This position has mandatory seasonal Influenza vaccination requirements and is subject to annual seasonal Influenza vaccinations unless otherwise exempted for medical or religious reasons (documentation of exemption must be provided upon hire).
  • Tuberculosis (TB): This position requires the incumbent to have Tuberculosis testing.
  • This position requires the incumbent to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
  • This position is a Testing Designated Position subject to pre-employment screening and random drug testing. Selectees will be required to consent to participation in random drug urinalysis testing.
  • Driver's License: The duties of this position require the incumbent to possess or obtain and maintain a valid state Driver's License in one of the 50 U.S. states or possessions to operate vehicles.
  • This position is subject to TDY 10% or business travel requirements to various locations. This may require travel via privately owned vehicles, commercial transport platforms, and/or military aircraft, ships, and vehicles.
  • Lifting: Lifting up to 15 pounds may be required.
  • This position is required to work uncommon tours of duty hours such as, on-call, working either fixed or rotating shifts, evenings, weekends and/or holidays.
  • This position is designated "Mission Essential". In the event of severe weather conditions or other such emergency type situations (natural or man-made disaster).
Qualifications

In order to qualify, you must meet the education and/or experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Your resume must clearly describe your relevant experience; if qualifying based on education, your transcripts will be required as part of your application. Education/Experience: A diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program and possession of at least one full year of professional nursing experience (equivalent to the GS-9 grade level or above) which includes, 1) coordinate healthcare services at Military Treatment Facilities 2) serve as patient's liaison with healthcare professional across military and civilian sectors 3) complete follow up assessments in the electronic record with updated information of the patient's current condition.

OR

Education: A PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.

EXEMPTION: You are exempt from the qualification requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet one of the requirement below. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.
