Essential Duties & Responsibilities
The incumbent independently performs direct nursing care to clinic patients via telemedicine, virtual, and face-to-face care to include triage, procedures, nurse-managed protocols, and medication management.
Incumbent assists clinician with physical examinations, treatments, diagnostic procedures, and coordination of health care needs of patients.
Incumbent tracks patient's laboratory, radiology, and referral results to completion and tracks at-risk patients by following the plan of care and assisting in making future appointments.
Incumbent initiates and maintains medical history on patients, records observations, and stated symptoms for use by the clinician.
Incumbent observes and records pertinent vital signs and orders laboratory procedures on patients that may be required for clinician.
Incumbent assists in counseling and educating patients to include, but not limited to diet, personal hygiene, acute or chronic illness, wellness/preventive health measures, normal development, family planning, and immunizations.
Incumbent initiates emergency care as needed and assembles, operates, and monitors specialized equipment such as Electrocardiogram (ECG) machine, Nebulizer Spirometry, Pulse Oximeter machine, suction equipment and any other equipment used in the delivery of patient care.
Ensures administrative requirements related to patient's visit are completed timely and correctly.
Utilizes automated programs and information technology as effective communication and management tools to facilitate the patient experience and coordination of care.
Collaborates with the clinic manager and/ or department head to assist in directing, planning, coordinating, implementing, and evaluating activities related to the delivery of ambulatory nursing care.
Participates actively in team huddles; proactively assists team's identification of needed population health measures and required procedures during schedule scrub; reviews patient's plan of care with team members for coordination.
Acts as a patient advocate by facilitating communication for the patient with other departments and their staff as required. Maintains effective communication with staff, patients, families, and between other departments.
Promotes a safe working environment.
Collaborates with other clinic team members in improving documentation of the episode of care using the Military Health System (MHS) and
DHA-approved standardized documentation templates in the Electronic Health Record (EHR) enabling measurement and analysis of nursing care, and linking patient complexity, nursing care needs, and nurse-sensitive outcomes.
Incumbent identifies new clinical requirements and takes necessary action to ensure changes and adaptations are implemented.
Collaborates with the clinic manager and/or other clinic leadership to identify lacking competency among team staff and assists with re-training as required.
Provides and participates in continuing education and training as appropriate for all levels of ancillary team staff and other nurses within the assigned medical setting. Collaborates with the clinic manager and/or other clinic leadership to identify problems among the PCHM team to help address concerns.
Provides for orientation of new personnel; ensures duties of the clinic team commensurate with their abilities utilizing the orientation program.
Assists in the development of standards within the clinic directed at defining staff performance in ambulatory nursing activities.
Participates in periodic in-service programs for the continuing education of staff.
Plans and implements performance improvement (PI) activities based on the PI plan for the command.
Participates in and/or manages quality improvement activities conducted within the clinic.
Coordinates schedules with other team members to ensure adequate access to care for the clinic team's patients.
Perform a full range of RN duties in accordance with assignment including: triage; patient assessment and monitoring; use of patient monitoring and treatment equipment; appropriate nursing care, procedures, and treatments; execution of physicians' orders within the guidelines of standard nursing practice; documentation of patient care and observations; and patient education and emotional support.
Assess each patient and perform triage duties as assigned.
Formulate and carry out a goal-directed plan of care which based on determined nursing diagnosis and patient outcomes and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies.
Evaluate effectiveness of self-care given by all health team members, and contribution of systems, environment, and instrumentation in progressing patient toward outcomes.
Provide treatment and discharge instructions upon patient release.
Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status.
Provide a safe and clean environment for each patient.
Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.
Assist in planning, provide clinical guidance and provide instruction to LPNs/LVNs, nursing assistants and ancillary personnel.
Demonstrate sound knowledge base and action in the care and decision making for designated patient populations. If weaknesses or deficiencies are identified either by peer reviews, quality control reports (QCRs), or evaluations the HCW will be required to complete refresher training (in-services, competency reviews, continuing education, etc.) at Contractor expense.
Perform documentation (manually and/or by use of computer) duties on unit and in- patient record that are timely, accurate, and concise.
Seek validation of knowledge base, skill level, and decision making as necessary and assertively seek guidance in areas of question.
Implement care in a knowledgeable, skillful, consistent, and continuous manner.
Support priorities of patient care based on essential patient needs and available unit, command, and community resources including time, personnel, equipment, and supplies.
Demonstrate competency in medication administration orally, intramuscular (IM) and subcutaneous (SQ) and demonstrate competency in phlebotomy as specific to the position.
Safely perform urinary catheters among all ages.
Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.
Perform documentation (manually and/or by use of computer) duties on unit and in- patient record which are timely, accurate, and concise.
Participate in clinical activities at the unit, directorate, and command level, such as participating in formal and informal classes, assisting in training new paraprofessional personnel with the goal of helping them successfully complete competency-based skills task lists and participating in training initiatives.
Demonstrate self-directed learning.
Participate in the development and attainment of unit and Nursing Directorate goals.
Demonstrate effective and professional communication methods and skills, using lines of authority appropriately.
Formulate and use effective working relationships with all health care team members, patients, and significant others. Refer unsolved complaints or infractions to the Division Head with recommendations for appropriate action.
Practice effective problem identification and resolution skills as a method of sound decision making.
Remain flexible in staffing patterns and resolution of staffing conflicts; participate in temporary assignment measures.
Provide individual, group, and family/significant other counseling and health teaching in relation to the patient's condition and his/her ability for self-care. Provide for accessibility to various literature resources dealing with subjects related to specific patient health problems. Provide physical and psychological support to patients and significant others, explain procedures and treatments, and promote cooperation among staff, patients, and significant others.
Participate in the orientation and training of newly assigned unit personnel. Participate in training initiatives.
Provide input into competency assessments of newly assigned unit personnel.
Collaborate in the review of clinic standing operating procedures.
If directed, perform phone triage based on accepted protocols and assist as needed in accessing appropriate ambulatory/emergency care for beneficiaries.
Provide training and/or technical guidance as applicable to supporting Government employees (i.e. hospital corpsmen, technicians, students) assigned to the HCW during the performance of clinical procedures. Such guidance and interaction will adhere to Government and professional clinical standards and accepted clinical protocols.
Education & Experience Requirements
Possess a minimum of one-year full-time experience as a Pediatric Registered Nurse in a direct care setting.
Possess a Baccalaureate Degree in Nursing, a Graduate Diploma in Nursing, or an associate degree in nursing, from an accredited agency such as the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
Possess and maintain a current unrestricted license to practice as a registered nurse in any one (1) of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
Prior to commencing services at Naval Hospital Okinawa, the RN shall attend a 2- hour lecture on IV certification. Following the lecture, the RN shall obtain certifications (examples: IV, medications and blood administration) in accordance with clinic nursing service and command policies within 60 days following the commencement of services, including MTF observation of three (3) completed IV sticks during this time.
Possess a current Basic Life Support certification.
Provide two letters of recommendation written within the last two years attesting to clinical skills. A minimum of one of the letters must be from a supervisor. The other letter must be from either a clinic or hospital administrator, or a practicing physician. Reference letters shall attest to the quality and quantity of experience. The letters may also address patient rapport and the communication skills between practitioner and patient and among peers. Recommendation letters must include name, title, phone number, date of reference, address and signature of the individual providing reference.
Working Conditions/Working Environment/Physical Demands
The HCWs shall provide services Monday through Friday between the hours of 0600 and 1800. The HCW shall be assigned an 8 to 12 hour shifts, (to include an uncompensated .5 to 1 hour meal break depending on shift length). In no instance will the HCW be required to provide services in excess of 80 hours per 2-week period. The specific schedule for each HCW for each 2-week period will be scheduled 1 month in advance by the supervisor.
Work is primarily performed indoors at a Military Treatment Facility; employee shall be physically capable of standing and/or sitting for extended periods of time, have the physical ability to perform assigned tasks, able to lift twenty (20) pounds.
Must be a United States citizen, successfully pass an extensive background investigation and receive government client approval.
