Basic Requirement for Behavioral Health Case Management:
Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position. This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Specialized Experience: One year of specialized experience which includes conducting behavioral health assessments/interviews; coordinating behavioral health care plans with a multidisciplinary team; and evaluating appropriateness, quality, or cost effectiveness of care. This definition of specialized experience is typical of work performed at the second lower grade/level position in the federal service (GS-11).
Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted.

Help
- Serve as a behavioral health case manager on a multidisciplinary medical team, conducting complex assessments and coordinating care for high-risk patients.
- Develop and implement individualized treatment plans, monitor medication effects, and provide psychoeducation, skills training, and therapeutic interventions.
- Facilitate care transitions and collaborate with internal and external providers to ensure continuity and quality of care.
- Lead weekly treatment planning meetings, analyze patient data to identify trends, and recommend program improvements.
- Analyze patient data to uncover trends, guide program improvements, and support smooth transitions between levels of care.

- Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
- A Personnel Security Investigation may be required.

- Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross or American Heart Association. Advanced certification (e.g., ALS) does not supersede BLS.
- This position may require the incumbent to work during irregular work hours which may include evenings, weekends, holidays, and/or varying work schedules.
- This position may require rotating shifts (i.e. work schedule that requires employees to continuously rotate from one shift to another at predetermined intervals).
- This position may require the incumbent to work on call during irregular work hours which may include evenings, weekends, holidays, and/or varying work schedules.
- This position may require the incumbent to work weekends and/or holidays.
- Must be able to obtain and maintain a valid state Driver's License in one of the 50 U.S. states or possessions to operate vehicles
- Position may be designated as Mission Essential. The incumbent will be required to report to work during times of contingency operations, natural disasters, extreme weather or other emergency situations.
- Appointment to this position is subject to a one year probationary period unless the appointee has previously met the requirements as described in 5 CFR Part 315. While probationary the incumbent can be terminated with limited appeal rights.
