Overview

The EMT Paramedic (EMT-P) serves as the onboard medical authority while providing advanced life-support care, preventative health screening, and emergency response throughout at-sea operations. This position exists to safeguard the health and readiness of all embarked personnel by ensuring continuous medical coverage in a remote, maritime environment. The EMT-P manages medical inventories, maintains health records, and coordinates with a remote medical director for telemedicine guidelines, ensuring compliance with HIPAA and the client's safety standards. The role's ultimate purpose is to keep the client mission ready and medically self-sufficient during all phases of operation.

This position performs all duties and responsibilities in accordance with the Mission, Vision, and Core Values of Cayuse.

Responsibilities

  • The EMT Paramedic provides continuous medical readiness, emergency response, and preventative health support. Responsible for safeguarding the health of all embarked personnel, the EMT-P ensures medical preparedness before, during, and after each mission, delivering Advanced Life Support (ALS) care in compliance with our client and HIPAA/PII standards.
  • Perform pre-mission medical screenings, record medical histories, and maintain protected health documentation.
  • Deliver acute and emergent care for all crew and embarked personnel following Advanced Cardiac Life Support protocols.
  • Maintain all prescription medications, consumables, and medical equipment required for underway operations
  • Coordinate with the contracted medical director to provide 24/7 telemedicine and reach-back medical consultation.
  • Participate in pre-underway safe way briefings and at-sea safety drills, briefing crew on medical response procedures.
  • Monitor crew health daily, observing conditions and fitness to support mission success.
  • Operate and maintain onboard medical equipment such as AEDs, EKGs, and trauma kits to ensure operational readiness.
  • Generate medical documentation and reports, including Medical Incident Reports and Post-Mission Trip Reports, in accordance with the client's requirements.
  • Inventory all medical supplies pre- and post-mission; recommend restock levels and in-port medical kit requirements.
  • Ensure compliance with the client's medical privacy regulations and maintain strict control of patient information.
  • Support the Officer in Charge (OIC) in emergencies by establishing medical treatment areas and coordinating potential medical evacuations.
  • Review medical Standard Operating Procedures (SOPs) for accuracy and recommend updates as required.
  • Other duties as assigned.


Qualifications

  • Must hold a current State of Hawaii Emergency Medical Technician - Paramedic (EMT-P) license or National Registry certification at the Paramedic level
  • Must maintain all credentials required to practice under the applicable Hawaii or federal authority throughout the contract period.
  • Must possess valid Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Advanced Trauma Life Support (ATLS) certifications.
  • Minimum 10 years of experience as a working Paramedic; 15 years preferred, with demonstrated competence in emergency medical services in remote, maritime, or expeditionary requirements.
  • Experience supporting austere or at-sea operations, coordinating with flight/shipboard evacuation or telemedicine providers, strongly desired.
  • Proficiency in Advanced Life Support (ALS) and trauma management, including airway control, IV/IO therapy, intubation, cardiac monitoring, defibrillator, and medication administration.
  • Ability to manage and track controlled substances, medical supplies, and consumables per Government standards.
  • Skilled in maintaining and operating shipboard medical equipment such as EKGs, AEDs, oxygen systems, and portable monitors.
  • Competence in HIPAA and Personally Identifiable Information (PII) protection, recordkeeping, and medical documentation management.
  • Capable of independent decision-making in remote settings, maintaining communication with a medical director 24/7 telemedicine support.
  • Working knowledge of USCG emergency-response requirements and ability to integrate with crew drills.
  • Must be physically fit to serve aboard a vessel at sea for up to 21 consecutive days.
  • Must pass pre-employment background check and meet maritime medical-fitness standards.
  • Must be able to pass a background check. May require additional background checks as required by projects and/or clients at any time during employment.

  • Must be self-motivated and able to work well independently as well as on a multi-functional team.
  • Ability to handle sensitive and confidential information appropriately
  • Proficient in MS Office, Word, Outlook, PowerPoint, and Excel.


Desired Qualifications:
  • CPR/AED Instructor certification desirable
  • Familiarity with Government safety, emergency-response, and reporting protocols preferred.
  • Experience supports austere or at-sea operations, coordination with flight/shipboard evacuation or telemedicine providers, strongly desired.
  • SECRET clearance is not required but preferred.
