Position Description

The Complex Case Manager - Cerner Community Care will support Veterans and high-need populations by evaluating, optimizing, and supporting workflows across Cerner's Community Care platform. This full-time role focuses on helping clinical teams improve care coordination for patients with complex medical, behavioral, and social needs. The individual in this role will work through change tickets and issue resolution processes, applying deep knowledge of available Veteran resources and system functionality to ensure that workflows and configurations support access, safety, and quality outcomes. Applicants must have exceptionally strong communication skills and thrive in an environment of leading discussions and meetings with executive-level stakeholders.

Type of Employment: Full-Time

Work Location/Travel: Expected 50% travel required; applicants must be free from travel restrictions to accommodate evolving needs of the client.

Salary Range: $83,000 - $96,000 base salary, depending on experience and location. Competitive salary/pay, potential for bonus. There are multiple factors that are considered in determining final pay for a position, including, but not limited to, relevant work experience, skills, certifications and competencies that align to the specified role, geographic location, education and certifications as well as contract provisions regarding labor categories that are specific to the position. The pay range for this position is highly competitive but dependent upon employment type (1099, contract, W2).

Responsibilities:
• Assess and triage open change requests and issues and apply industry knowledge and expertise of Cerner functionality to resolve the tickets.
• Serve as an advocate for end users by identifying gaps and advancing opportunities to better support Veteran care using community-based services.
• Demonstrate excellent communication skills and present in the most polished and professional manner.
• Collaborate with end users and technical teams to remove workflow barriers and enhance care delivery capabilities.
• Evaluate documentation processes and recommend configuration updates to support complete, efficient, and standardized information capture.
• Ensure Cerner Community Care modules are optimized to reflect the realities of complex care coordination without placing undue burden on end users.
• Provide workflow support and subject matter expertise for all care management services.
• Assess workflows and operational processes used by clinicians and case managers and provide recommendations for enhanced system alignment.
• Understand the care coordination needs of high-risk Veterans and ensure system functionality and configuration support their access to care.
• Apply expert-level knowledge of complex care planning to evaluate current-state workflows and recommend optimizations using Cerner Community Care.

Knowledge and Skills Required:
• Hands-on experience using Cerner Community Care for case management and care coordination.
• Expertise in managing patients with complex physical, behavioral, and social needs.
• Knowledge of VA Community Care programs and referral management processes.
• Strong documentation, organizational, and time management skills.
• Comfort working across interdisciplinary teams and escalating complex situations.

Education/Certification Required:
• Bachelor's degree in Nursing, Social Work, or related clinical field required.
• Master's degree in Nursing, Public Health, Social Work, or Health Informatics preferred.
• Clinical licensure required (e.g., RN, LCSW, LMSW).

Experience Required:
• Minimum of 5 years of clinical case management experience.
• Minimum of 5 years of hands-on experience with Cerner implementations including Community Care.
• Required experience supporting VA and/or DoD Cerner deployments.

Technical Skills Required:
• Proficiency in Cerner Community Care case management modules and care plans.
• Understanding of HL7 interoperability, referrals, and community network documentation.
• Competency using Microsoft Office Suite and collaborative platforms.
