Position Description
The Population Health - Cerner Systems expert will support optimization and strategic use of Cerner's population health and analytics platforms to improve care delivery and outcomes for Veterans. This role evaluates and informs how Cerner tools (such as HealtheIntent, HealtheRegistries, and HealtheCare) can be configured, integrated, and leveraged to identify gaps in care, track performance, and coordinate services across care teams. This position requires deep knowledge of value-based care principles, quality reporting requirements, and system capabilities that enable scalable interventions across populations. Applicants must have exceptionally strong communication skills and thrive in an environment of leading discussions and meetings with executive-level stakeholders.

Type of Employment: Full-Time
Work Location/Travel: Expected 50% travel required; applicants must be free from travel restrictions to accommodate evolving needs of the client.
Salary Range: $92,000 - $108,000 base salary, depending on experience and location. Competitive salary/pay, potential for bonus. There are multiple factors that are considered in determining final pay for a position, including, but not limited to, relevant work experience, skills, certifications and competencies that align to the specified role, geographic location, education and certifications as well as contract provisions regarding labor categories that are specific to the position. The pay range for this position is highly competitive but dependent upon employment type (1099, contract, W2).

Knowledge and Skills Required:
Education/Certification Required:
• Bachelor's degree in Public Health, Health Informatics, Nursing, or a related field required.
• Master's degree in Health Informatics, Public Health, or Healthcare Administration preferred.
Experience Required:
• Minimum of 5 years of experience working with Cerner population health tools or similar analytics platforms.
• Required experience supporting VA and/or DoD Cerner deployments.
• Strong background in population health strategy, quality measurement, and analytics integration.
Technical Skills Required:
• Proficiency in Cerner HealtheIntent, HealtheRegistries, and data mapping functions.
• Experience with data extraction, cohort creation, and registry logic for population health initiatives.
• Proficiency with Microsoft Office Suite and collaboration platforms such as Teams and SharePoint.

Responsibilities:
- Serve as a subject matter expert on Cerner's population health tools, including HealtheIntent, HealtheRegistries, and related data services.
- Evaluate current-state workflows and data flows to identify areas of misalignment with population health goals.
- Assess and triage open change requests and issues, and apply industry knowledge and expertise of Cerner functionality to resolve the tickets.
- Recommend enhancements to registry and patient cohort designs to support risk stratification, screening compliance, and chronic disease management.
- Collaborate with clinical and operational teams to integrate quality metrics and registry logic into actionable workflows.
- Align data reporting functionality with clinical program requirements for VA/DoD initiatives and other federal performance programs.
- Support configuration and workflow documentation that enables data-driven care planning and care gap closure.
- Evaluate technical and functional aspects of Cerner tools to support scalable care team engagement and population segmentation.
- Provide at-the-elbow support and learning.
- Demonstrate excellent communication skills and present in the most polished and professional manner.
Knowledge and Skills Required:
- Deep knowledge of Cerner HealtheIntent platform and related population health registries and reporting tools.
- Understanding of risk stratification models, value-based care, and population health frameworks in federal healthcare environments.
- Ability to bridge clinical, operational, and technical perspectives to support workflow optimization.
- Proficiency in stakeholder facilitation and cross-disciplinary collaboration.
- Ability to analyze population health data to inform system improvements and care delivery outcomes.
