Spectrum Healthcare Resources has an Exciting Potential Opportunity for Inpatient and Outpaitient Medical Record Technician (Coder) to support Central Texas Veterans Healthcare System in Temple,TX.
Requirements:
- Must be U.S. Citizen
- High School degree/GED
- Successful completiton of the following: Registered Health Information Technician (RHIT), Registered Health Information Administrator (RHIA), Certified Coding Specialist (CCS), Certified Coding Specialist-Physician (CCSP), and/or Certified Professional Coder (CPC). Coding certification. Must be proficient in the following code sets: ICD-10 CM, CPT, and HCPCS coding and be able to provide medical record coding.
- Must have 3 years continuous coding experience
- Shall be able to read, write, and speak English well enough to effectively communicate with patients and other staff members.
- Have knowledge of computer operations and proficiency in the use of basic word processing, data entry, and automated records.
- Must have documented experience in the medical field and in work acting in such a capacity.
Job duties:
- Code and/or review outpatient and inpatient encounters including primary care, specialty services, surgery, ambulatory procedures, endoscopy, and facility inpatient care
- Perform 100% data validation of assigned encounters, including outpatient, inpatient, surgical, prosthetic, and diagnostic services
- Apply ICD-10-CM, ICD-10-PCS, CPT, HCPCS, and Evaluation & Management coding as required
- Ensure accurate documentation and coding to support diagnoses, procedures, complications, and co-morbid conditions
- Review electronic and paper records to confirm services are properly documented, coded, and sequenced according to official guidelines
- Communicate with clinicians (email or in person) for clarification regarding documentation and clinical details
- Maintain accuracy of diagnostic and procedural data to support correct reimbursement and reporting
- Abstract medical, surgical, laboratory, pharmacy, radiology, demographic, and administrative information from medical records
- Identify all conditions and services that affect reimbursement and ensure they are recorded ethically and accurately
- Determine proper coding for routine as well as new, unusual, or complex diagnoses and procedures
- Perform concurrent coding when needed
- Query clinicians to resolve unclear or ambiguous documentation and close queries within required timeframes
- Maintain written documentation of reviews and provide to the Contract Representative as required
- Stay current on regulatory and compliance updates from CMS and other governing entities
- Maintain an accuracy rate of 95% or higher; coding is subject to ongoing audit and review
- Follow federal and state guidelines and report any compliance or conflict issues immediately
- Maintain credentials, competency documentation, qualifications, and continuing education records as required
